Supported Independent Living in Diamond Creek can help participants manage daily routines with reliable support while remaining connected to a familiar Nillumbik community. Our SIL services are tailored around individual goals, preferences and NDIS plans, with assistance for personal care, household tasks, life skills, community participation and overnight needs.
Supported Independent Living (SIL) is an NDIS funded support that provides regular assistance with everyday tasks in a participant’s home. It can be delivered in shared living or an individual home setting.
Support may be available during the day, evening, overnight or across 24 hours depending on assessed needs and approved funding. SIL covers the assistance provided in the home rather than rent or normal household expenses.
Participants remain involved in decisions about their routine and support, helping daily life remain personal and familiar.
SIL may suit participants who:
Diamond Creek’s town centre, railway station and local community facilities provide practical opportunities for participants to build confidence with shopping, transport and everyday activities while remaining close to home.
We assist with showering, dressing, grooming, toileting, mobility, transfers, morning and evening routines and medication reminders.
We support groceries, cooking, meal planning, cleaning, laundry and home organisation. Participants can practise these tasks where skill development is a goal.
We assist with budgeting, meal preparation, appointment planning, communication, decision making and public transport use. Diamond Creek Station and the local town centre provide useful settings for building travel and shopping confidence.
We support local recreation, appointments, social activities, shopping, hobbies and visits with family or friends across Diamond Creek and nearby Nillumbik communities.
Where required and funded, support can include evening assistance, sleepovers, active overnight care, monitoring and 24 hour support.
We use a clear process to understand the participant and establish suitable support.
Initial conversation
We discuss goals, preferences, current supports and the living arrangement being considered.
Support needs review
We review the NDIS plan, daily support requirements and relevant information from the participant’s support team.
Personalised planning
We develop a plan around routines, communication needs, goals and required assistance.
Service agreement
We document the agreed support clearly before services begin.
Ongoing reviews
We review support regularly and adjust the plan when needs or goals change.
We work with families, guardians, support coordinators, plan managers and allied health professionals where appropriate.
Planning discussions, progress updates and regular reviews help keep support consistent while the participant remains at the centre of decisions.
Support coordinators working in Diamond Creek and surrounding Nillumbik communities are welcome to contact us about referrals.

Our Diamond Creek SIL support combines reliable assistance at home with practical opportunities to build confidence in familiar local settings.
